I think one of the worst mistakes in B7 was to bring back Servalan in the guise of Sleer (much as I enjoy Jacqueline Pearce's acting).
Servalan must have been the most well-known person in the Federation, if not the whole galaxy.
This probably ties into the S4 clothes but, evidently, people in the B7 universe have the hardest time recognizing people once they've had a major wardrobe change. Apparently, when Servalan isn't wearing at least some white, she needs to tell people who she is. Hence, on Chenga, while wearing Dayna's clothes, she had to make a big fuss to get properly identified.
As for S4, I suppose, after losing Liberator, the others wanted to a bit more low profile and chose clothes no one could recognize them in. Avon, who was already showing signs of stress, evidently didn't care.
As for why Servalan and Avon never seemed to have these recoginition problems, that was just one of those extraordinary skills we expect in them.
It's true Dayna never had this problem either, but she probably ran computer scenarios night and day on ways Servalan might try to keep Dayna from killing her and how to overcome them.
Or maybe people in the B7 universe are actually more smell sensitive than we've realized and they relied on odor to ID people. Servalan changed her perfume. Since it was the perfume Avon had given her, he could still recognize her. Since the musk base came from something Dayna had killed, she could recognize her (perhaps they really did rescue Ensor's fish, which were actually the pollywog stage of those other critters and she'd been using that smell for their track and attack training, but they went down with the Liberator, messing up another perfectly good plan).
And why didn't Avon blow her cover?
